New Data on Diabetes Management: Heavy Reliance on CDEs
Certified diabetes educators (CDEs) and case managers are key influencers in diabetes management, according to a 2011 survey on diabetes management strategies by the Healthcare Intelligence Network (HIN).
Weight management is the greatest challenge of managing diabetes, said the majority of respondents to the December survey, and more than three-quarters 77 percent of healthcare organizations said they are taking a disease-specific approach to improving health outcomes and self-management in patients and health plan members with diabetes.
Almost half of respondents said they review claims data to identify individuals who could benefit from high-touch diabetes management.
