New Webinars to Explore Partnerships in Patient-Centered Care: The Virtual Medical Home and the Accountable Care Organization
Two new sessions in the Healthcare Intelligence Network's Medical Home Open House Series will explore two partnerships that expand care delivery and boost profitability through the patient-centered medical home: the virtual medical home and the accountable care organization (ACO).

SEA GIRT, NJ, USA — Healthcare organizations invested in the patient-centered medical home (PCMH) can benefit from two collaborative models that not only expand the range of services offered but also increase profitability — the virtual medical home and the accountable care organization (ACO). Two new webinars in the Healthcare Intelligence Network's Medical Home Open House Series will explore these emerging models through the experiences of early adopters of the virtual medical home and ACOs.
The ACO — a network of primary care physicians, one or more hospitals and subspecialists that provide patient-centered care — provides a financial framework for patient-centered care delivery, while the virtual medical home offers a more complete array of services (such a behavioral health) across the healthcare continuum through a series of community, public or private partnerships in a way that is seamless to patients and health plan members.

Session 1: Coordinating a Virtual Medical Home in Your Community: Lessons from the Iowa Collaborative Safety Net Provider Network on 9/23/10 at 1:30 pm Eastern. In this 45-minute webinar, Sarah Dixon Gale, lead contract manager, Iowa/Nebraska Primary Care Association and Michelle Stephan, chief executive officer, Siouxland Community Health Center, will share how the primary care providers in this community work with other local, community-based organizations, as part of a virtual medical home, to improve access to and the quality of care.

Session 2: The Colorado Accountable Care Collaborative: Practical Lessons from an ACO on 9/29/10 at 1:30 pm Eastern. With a January 2011 go-live date for an accountable care organization pilot, the Colorado Department of Health Care Policy and Financing is entering into the final stages of an RFP process to identify regional organizations that will function as ACOs, the medical homes that will serve as providers within the ACO and a state-wide data and analytics vendor that will provide real-time data to the providers within the ACO. During this 45-minute webinar, Laurel Karabatsos, deputy Medicaid director and Jerry Smallwood, Medicaid reform unit manager, will walk participants through the ACO development process in Colorado.

Webinar Formats: Each session includes a 15-minute Q&A; “On-Demand” rebroadcast available two days after the session; 45-minute training DVD or CD-ROM with printed transcript available three weeks later. Participants may add a DVD or CD to their live session registration to share with colleagues.
The sessions are the latest in HIN's ongoing Medical Home Open House series, which presents key topics in PCMH implementation — staffing, care delivery, patient education, case management, reimbursement, medication management and more.
