Background on the Medical Home: In the medical home model, the patient's physician leads a team that collectively takes responsibility for the patient's ongoing care, which is coordinated across hospitals, nursing homes, insurers and the patient's family and community. Initially advanced by pediatricians in the 1990s, the model is gaining ground throughout primary care. The establishment of medical homes figures prominently in efforts to reduce improper use of costly hospital emergency rooms and has prompted providers to seek revisions in reimbursement models to match this new model of care.
