  • Background on the Medical Home: In the medical home model, the patient's physician leads a team that collectively takes responsibility for the patient's ongoing care, which is coordinated across hospitals, nursing homes, insurers and the patient's family and community. Initially advanced by pediatricians in the 1990s, the model is gaining ground throughout primary care.

  • Video Highlights: In the first pilot of the medical home model for diabetics, a data-sharing partnership between physician and payer nearly doubles patients' compliance with a key diabetes marker. In the second pilot, efforts to educate Medicaid patients with diabetes pay off in more cost-effective utilization of healthcare resources.
