How Hospitals Can Benefit from the Medical Home Model
During a December 17, 2008 webinar, the director of a hospital-based medical home network will detail how its patient-centered approach has reduced hospital admissions and ED visits and improved quality of care and health status for its low-income and uninsured populations.

MANASQUAN, NJ, USA — The benefits reaped by the patient-centered medical home (PCMH) model of care are by no means limited to primary care practices, as evidenced by the success of the medical home network in place at Palmetto Health, a three-hospital system in South Carolina. Marcus Barnes, director of the Richland Care Medical Home, will describe the 10-year journey that expanded the PCMH to a hospital system and the benefits they have gained, including fewer hospital admissions and ED visits and enhanced outcomes and quality of care for the population it serves.
