"Our April 2009 e-survey on the effect of care transition programs also found that, like Geisinger Health Plan, most care transition programs (67.5 percent) target the Medicare population. A focus on transitions between care sites can yield many benefits. According to surveyed healthcare organizations with transitions of care programs in place, 57.5 percent have found them to reduce readmissions and more than three-quarters say they increase patient satisfaction. Forty-five percent told us they see a reduction in ER visits, 42.5 percent have reduced cost, 40 percent have shortened lengths of stay and slightly more than a third (37.5 percent ) have avoided admissions."
