care_transitions
About 65 percent of respondents use care transition management to prevent hospital readmissions, compared to 62.3 percent in 2009.

January 12, 2011

SEA GIRT, NJ, USA —Inappropriate and preventable rehospitalization is not only an inefficient use of healthcare resources, but also a waste of payors’ and consumers' healthcare dollars. According to "2010 Reducing Readmissions: Care Transition Management Remains Key," a complimentary downloadable HIN white paper, 65.2 percent of survey respondents are focusing on care transition management within their programs to reduce hospital readmissions, a slight increase since 2009 when it was 62.3 percent. The white paper summarizes HIN's second annual Reducing Hospital Readmissions survey, in which nearly 90 healthcare organizations described their efforts in reducing readmissions.
