Background: HIN launched ReadmissionsRx in response to increased requests from healthcare organizations for strategies to reduce the exorbitant costs of unnecessary hospitalizations as well as an increasing reluctance on the part of payors to reimburse providers for hospitalization they deemed unnecessary, explains Melanie Matthews, HIN executive vice president and chief operating officer.

Content: Each issue will deliver strategies to reduce hospital readmissions that encompass care plan development, case management, care transitions, pre- and post discharge planning, medication reconciliation and much more — with a special focus on reducing rehospitalizations among the Medicare population.
