Modeled after Geisinger Health System's Patient Care Navigator program, Bon Secours Health System launched an embedded case management program in 2009. Adapting the Geisinger model to fit its unique needs, Bon Secours' embedded case managers have been placed in eight physician practices and will be expanded system-wide over the next two years.
During "Embedded Case Management in the Primary Care Practice: Program Design and Results," a 45-minute webinar on August 10, 2011 sponsored by the Healthcare Intelligence Network, the VP and COO at Bon Secours Health System will share the program's development and roll-out strategy for embedding case managers.
