The Medical Home Open House webinar series sponsored by the Healthcare Intelligence Network is focused on improving continuity and quality of care for patients served by a patient-centered medical home, while providing financial returns for the providers and payors participating in the program.
The series, which began in June and continues with next week's program, Effective Case Management in the Medical Home, has covered the critical components of the medical home from contracting and reimbursement strategies to patient education and engagement to staffing and NCQA certification.
Here are some highlights from the series:
First, Barbara Wall, a healthcare consultant for several medical home pilots in the Northwest, says patient-centered care starts with a look at the appointment calendar. From the Medical Open House session “Patient Education and Engagement in the Medical Home,” Barbara describes how this daily look at the patient schedule can lead to a comprehensive patient education and outreach program.
Patient outreach is never more critical than when patients are moving from one care site to another, such as a discharge from hospital to home, or from hospital to a skilled nursing facility. These transitions often are fraught for medication errors and confusion, which is why Geisinger Health Plan care managers focus on medication adherence during care transitions. In the Medical Home Open House session “Constructing Care Transitions to Reduce Hospitalizations,” Geisinger’s Doreen Salek provides some details.
The medical home model was originally developed for children with special healthcare needs. The adult medical home also is designed so that an individual’s special needs — including chronic illness and behavioral health issues — get the special attention and care they require. During the Medical Home Open House session “Under One Roof: Integrating Behavioral Health and Primary Care,” healthcare consultant Liz Reardon describes this shift in focus.
