Constructing Care Transitions to Reduce Hospital Admissions will be held on 8/26/09 at 1:30 p.m. Eastern.

The Transitions of Care is a critical component of Geisinger Health Systems medical home program and a huge opportunity. During this 45-minute call, Doreen Salek, director business operations of health services, Geisinger Health Plan, will share the essentials behind the transition teams for a medical home pilot at Geisinger that reduced hospital admissions by 20 percent and overall healthcare costs by 7 percent while improving patient satisfaction and clinical outcomes.

Under One Roof: Integrating Primary Care & Behavioral Health in the Medical Home, the first program in the Medical Home Open House Summer Series, was held on 7/1/09. Recording available.

During this 45-minute call, presenters Laura Galbreath, Director of Policy and Advocacy for the National Council for Community Behavioral Healthcare (NCCBH), and Liz Reardon, president of Reardon Consulting and former Managed Care Director for Vermont Medicaid, presented on the joining of current developments in the patient-centered medical home model with evidence-based approaches to integration of primary care and behavioral health.

Person-centered healthcare homes are critical to address significant health disparities for people with serious mental illnesses, according to the NCCBH report, "Behavioral Health/Primary Care Integration and the Person-Centered Healthcare Home." The report advocates the introduction of general healthcare capacity within mental health organizations or nurturing of seamless partnerships between mental health and primary care providers.

Since 2002, Vermont has pioneered the development of medical homes for this population, using a grant from the Robert Wood Johnson Foundation to improve access to primary care for mental health consumers, create infrastructure to support effective integration of primary care and mental health systems and engage consumers in the design of integration initiatives.
