Manasquan NJ USA -- February 26, 2010: New sessions on shared savings and embedded case managers in the patient-centered medical home have been added to the Medical Home Open House webinar series.

Embedded Case Managers: Navigating Care Transitions, Gaps in Care and Patient Compliance will be held on 3/10/10 at 1:30 p.m. Eastern

Whether embedded in a primary care practice, hospital or nursing facility, embedded case managers are helping patients to navigate the healthcare system in terms of care transitions and compliance to care plans.

During this 45-minute webinar, Diane Littlewood, RN, Regional Manager of Case Management for Health Services, Geisinger Health Plan, will examine an embedded case manager program, from the factors that will help you determine if a program is right for your organization and deciding on the placement to defining roles and responsibilities for a program.

Ms. Littlewood will cover:

  • Working with physician practices to embed case managers to assist with care transitions and patient compliance;
  • Identifying which patients would benefit from this type of case management;
  • Skill sets, roles and responsibilities of the embedded case manager;
  • Case load management;
  • Coding and reimbursement for case management; and
  • Results from the Geisinger Health Plan's embedded case manager program.

Shared Savings in the Medical Home will be held on 3/31/10 at 1:30 p.m. Eastern

A shared savings approach for reimbursement can be an effective recruiting tool for high-quality providers to participate in medical home programs.

During this 45-minute webinar, David West, MD, Hospitalist, St. Mary's Hospital, Grand Junction, Colorado, will examine how to structure a shared savings agreement. Grand Junction, with its high-quality and lower healthcare costs, has been cited by many as an example of how health reform should be structured. A key component of its system is a shared vision and shared incentives.

Dr. West will cover:

  • Methodologies developed and being considered for shared savings;
  • Policies and procedures to guide shared savings;
  • Factors to consider in a shared savings arrangement;
  • Process and outcome measures;
  • Adjusting for risk in a shared savings model; and
  • Other factors to consider in a shared savings arrangement.
