When patient-centered medical homes first moved into view, patients could expect
to be assigned to their medical homes by their health plan, their provider or via
self-enrollment. However, as support for the patient-centered model has grown, so
has a preference for a hybrid model of patient assignment that improves patient
tracking and care delivery.

Medical Home Assignment Approaches: Evaluating Models for Patient Enrollment and Tracking In Medical Home Assignment Approaches:
Evaluating Models for Patient Enrollment and Tracking
,
two industry thought leaders explore the tradeoffs of
existing patient assignment methods and propose a
hybrid assignment system that draws upon the data,
experiences and preferences of key medical home
stakeholders.

Setting the scene for patient assignment in a managed care environment is
Dr. Anita Murcko, medical director of clinical informatics and provider adoption
with the Arizona Health Care Cost Containment System (AHCCCS), the state's single
Medicaid agency. With nearly 20 health plans participating in Arizona's managed care
system, Dr. Murko describes the medical home culture and infrastructure essential
to patient assignment and the financial and IT investments required to move an
organization toward 'Medical Home 3.0.'

Dr. Murko describes how AHCCCS strives to ensure that a participating health plan
shares its vision for patient-centered care delivery, including the details of the
medical home essay on its health plan RFP and the responsibilities it defines for
participating health plans, providers and contractors that will help shape AHCCCS's
patient assignment process going forward.

Providing the payor perspective is Dr. Charles DeShazer, market vice president of
clinical innovations at Humana. Dr. DeShazer cut his teeth on the medical home
model at Kaiser Permanente in Cleveland, where he helped implement the PCMH and
trained physicians in leading the medical home team. Now charged with leading
Humana's adoption of the PCMH, Dr. DeShazer describes Humana's hybrid model of
patient assignment and why patient assignment should matter to physicians,
patients and payors.

Detailing how existing infrastructures must evolve to support ownership and
accountability for the total and continuous patient care that is a hallmark
of the PCMH, Dr. DeShazer delineates current challenges to patient assignment
and strategies to overcome patient perceptions and technology limitations
that can hinder patient assignment.
