Should ER Staff Identify Patients for
Advanced Illness Care Coordination?

"When identifying patients in the ER, early identification is always a good thing, even if that begins in the ED as a decision. Such a decision can be made locally by a healthcare system, assuming that there’s a workflow in place and there are resources to make it happen. I would underscore that clinicians who are directly interacting at the point of care and who are caring for and seeing the patients 'live' are the best identifiers of patients who can benefit from services like Compassionate Care. The ED is a good place to start."
