How Does Florida Blue Use Patient-Physician Agreements to Engage Patients to Follow Doctors' Orders?

"We provide our physicians with a welcome package. In it, we have written a very generic patient-physician agreement, which explains what a patient-centered medical home (PCMH) is. By signing the agreement, the patient agrees to work with the physician to meet the goals agreed to between the two of them. We tell the physician to have the member sign it, and we ask that the physician sign it, too. It is a physician group decision; it's a generic form letter than they can change any way they want. They can brand it; they can add or delete elements as befits their practice. But we are highly recommending that there be some kind of written agreement between the patient and the physician."
