"Throughout our test intervention, especially for the integrated care team approach, we found that once we worked with our patient closely, they became appreciative. They liked the level of trust between the team and the patient and they like the connection, especially for the medical home models in the non-mental health test stream. The patients were able to be redirected to their medical home and the PCP then became more engaged. Some of the PCPs received positive feedback and appreciated the opportunity to work with the patient closely. Obviously through our integrated care team approach, we do bring in additional resources such as specialists and other social service needs that typically a disadvantaged population, such as the Medicaid managed care population, would often experience."

Dr. Mina Chang presented during Reducing Avoidable ER Visits by Medicaid Patients Through Quality-Based Interventions, a 45-minute webinar, during which she described how a collaborative effort in Ohio that is developing actionable interventions to address why Medicaid beneficiaries are most likely to use the ED inappropriately.
