Top Tools to Rank Readmission Risk This Week's Challenge: As private payors follow CMS's reimbursement and reporting leads, healthcare organizations are taking bold steps to reduce avoidable readmissions among their most vulnerable and high-utilization populations. What We Learned: The Healthcare Intelligence Network's Second Annual Reducing Hospital Readmissions e-survey conducted in December 2010 captured the details of how 90 healthcare organizations are reducing readmissions among their populations. The tools that survey respondents use to identify individuals at-risk for returning to the hospital are:

  • Risk stratification: 57.8 percent
  • Chart review: 46.7 percent
  • EHR: 37.8 percent
  • Health claims: 35.6 percent
  • Predictive modeling: 31.1 percent
  • Patient registry: 13.3 percent
  • Pharmacy claims: 13.3 percent
  • Other: 8.9 percent
