Who's Connecting HRAs
to Care Management?

Who's Connecting HRAs to Care Management? This Week's Challenge: Aggregate data from health risk assessments (HRAs) provides a roadmap for healthcare organizations to deliver health promotion and disease management interventions to targeted individuals — with the goal of improving clinical and financial outcomes. We wanted to see who is connecting HRA results to care management programs. Click here to view a printable version of the chart.

What We Learned: HIN's June 2010 Health Risk Assessments e-survey captured the trends in HRA use, from format and target audience to incentive use and impact. Most of the 116 survey respondents are connecting HRAs to care management programs:

  • Yes: 77 percent

  • No: 23 percent
