We wanted to see which conditions are targeted most by embedded case managers.

What We Learned: HIN's Healthcare Case Management e-survey conducted in January 2012 captured the efforts and contributions of case management from 153 healthcare organizations, and also drilled down to trends in embedded case management. According to survey respondents, the top five conditions targeted by embedded case managers are:
- Cardiovascular disease: 76.4 percent
- Diabetes: 74.5 percent
- Congestive heart failure: 69.1 percent
- COPD: 67.3 percent
- Heart disease: 65.5 percent
