What We Learned: HIN's fourth annual industry survey on healthcare case management, administered in May 2013, captured data on case management responsibilities, diagnosis most responsive to case management, challenges of embedded case management, and much more. According to the 118 healthcare companies who responded to the survey, the top five ways individuals are identified for case management are:
- Case management assessment: 62.2 percent
Provider referral: 56.7 percent
Claims data: 50 percent
Hospital census: 47.8 percent
Self/family referral: 40 percent
