Identifying Candidates for Palliative Care This Week's Challenge: Approximately 78 percent of respondents to the Healthcare Intelligence Network's first annual Palliative Care survey in February 2014 said they use physician referrals to identify candidates for palliative care. We wanted to see how else organizations identify candidates for palliative care.

What We Learned: HIN's industry survey on palliative care documented the efforts made in palliative care among over 220 healthcare companies, including the challenges in implementing palliative care, composition of the palliative care team, and much more. According to respondents, the top five ways to identify candidates for palliative care include:

  • Physician (phys.) referrals: 78.2 percent
  • Case management (mgmt.) assessment (assess.): 59.7 percent
  • Member/patient (pt.) referrals: 42.7 percent
  • Diagnoses set: 32.3 percent
  • Prediction (predict.) algorithm: 11.3 percent
