Top 5 Components of a Home Visit This Week's Challenge: According to nearly 86 percent of healthcare companies who responded to HIN's e-survey on home visits, medication reconciliation is the top component of a home visit. We wanted to see what else occurs during a home visit.

What We Learned: More than 150 healthcare companies responded to HIN's inaugural industry survey on home visits to describe how they employ home visits, including who conducts the home visit, how individuals are identified to receive home visits, the greatest challenges of a home visit, and more. According to respondents, the top five components of a home visit are:

  • Medication reconciliation: 85.5 percent
  • Clinical assessment: 78.3 percent
  • Patient/caregiver education: 77.1 percent
  • Fall assessment: 62.7 percent
  • Socioeconomic assessment: 61.4 percent
