This Week's Challenge: In order to avoid being penalized by CMS, hospitals are now reexamining the hospital discharge to ensure the transition out of the hospital and the reception into the next site of care is a successful one. We wanted to see what the barriers are to reducing readmission rates.

What We Learned: HIN's third annual survey on Reducing Hospital Readmissions documented the highest rates of targeted programs to reduce readmissions in the survey’s three-year history. The survey captured details on these programs to curb readmission rates, along with the conditions most likely to trigger readmissions and much more. According to survey respondents, the top five barriers to reducing hospital readmission rates are:
- Patient handoffs: 20 percent
- Other: 18 percent
- Medication adherence: 16 percent
- Access to primary care: 12 percent
- Health literacy: 10 percent
