This Week's Challenge: Applying lessons learned from an employee-based population health management (PHM) program, Adventist Health is now rolling out a PHM program among its patient population. We wanted to see what goes into the initial evaluation during a member's first encounter in this program at White Memorial Medical Center, part of Adventist Health.

What We Learned: "The initial evaluation is very comprehensive. With the first encounter, you want to make sure that you get the current member's health status. You have to obtain a health history, a medication history, and learn if there are any ongoing medical issues. You have to assess the patient's medication adherence and find out if they are adhering to the medicine or not. You want to also assess their functional status and activities of daily living (ADL). You need to determine if the participant has the ability and expectations, and also the understanding, of being confident enough to manage their chronic disease. The participants also need to have the willingness to change; many times we've gotten to this point and they're not willing to change. You can't engage patients or participants that are not willing. Assessing their need is also part of the evaluation; do they have social needs, cultural needs, linguistics, and financial limitations?"
