What We Learned: HIN's healthcare case management survey captured data on case management responsibilities, diagnosis most responsive to case management, how individuals are identified for case management, and much more. According to the 118 healthcare companies who responded, the top five ways case managers interact with patients and members are:
- Telephone: 83.1 percent
- Face to face in care setting: 61 percent
- E-mail: 32.5 percent
- Home visits: 29.9 percent
- U.S. mail: 27.3 percent
