Sea Girt NJ USA -- August 1, 2011: Modeled after Geisinger Health System's Patient Care Navigator program, Bon Secours Health System launched an embedded case management program in 2009. Adapting the Geisinger model to fit its unique needs, Bon Secours' embedded case managers have been placed in eight physician practices and will be expanded system-wide over the next two years.
During Embedded Case Management in the Primary Care Practice: Program Design and Results, a 45-minute webinar on August 10th, Robert Fortini, vice president and chief clinical officer at Bon Secours Health System, will share the program's development and roll-out strategy.
