Case Management's Population of Impact
CareOregon’s case management philosophy has been guided by developing programs that are not disease specific, according to Rebecca Ramsay, BSN, MPH, senior manager of care support and clinical programs at CareOregon.
Instead, CareOregon has developed a program that provides complex case management to its members that have comorbid chronic conditions as well as a lot of social morbidity, Ramsay explained.
“Within that we certainly have conditions specific protocols that our case managers follow but we really think about our members in a broad bio-psychosocial context,” she added. “We pay a lot of attention to their assets and we identify those as being critical to developing a successful intervention with our members. We have also spent a lot of time developing proactive technology and outreach capabilities designed to identify and outreach to members earlier.”
Ramsay shared with participants in last month's webinar Case Management: Identifying, Monitoring and Managing Target Populations, how CareOregon is stratifying members in need of case management into a multi-disciplinary team through an overlapping identification methodology, which helps to overcome some of the limitations of predictive modeling and claims-based identification.
