In addition to its clinical case managers, CareOregon has been building its
case management program capacity over the last five years to include
non-clinical staff members to fill the role of health guides for its
Medicare and Medicaid members. Members in need of case management are
stratified into this multi-disciplinary team through an overlapping
identification methodology.
