Sponsored by: Managing Transitions to Care for the Dually Eligible Medicare and Medicaid Patient

There are a number of strategies that can reduce the number of hospital admissions and re-admissions among the dually eligible Medicare and Medicaid population, ranging from programs that identify potential pharmaceutical interactions to in-person and/or telephonic coaching. During this 90-minute audio conference, we will examine what strategies are most effective in managing these transitions and the impact these programs can have on admission rates, length of stay and re-admission rates. Managing Transitions to Care for the Dually Eligible Medicare and Medicaid Patient, a February 14, 2007 audio conference, will examine effective care management strategies for this unique population.

2.) Prescription for Primary Care: Initiatives for Change

There are rumblings about the state of primary care at each step of the healthcare continuum: hospitals and health plans want primary care physicians (PCPs) to accept a larger role in disease management (DM) and care transitions, including patient education and follow-up. Meanwhile, PCPs struggle to provide quality care and devote adequate time to patients in the face of reduced reimbursements and increased reporting. It’s a scenario that frustrates patients, who increasingly turn to emergency departments (EDs) for primary care when the PCP is unavailable. In its October 2006 online survey, the Healthcare Intelligence Network (HIN) asked healthcare organizations how they are affected by the state of primary care and how they are handling the deficiencies.

3.) HealthPartners Reports DM And Generics Reduce Expenses Nearly $70 Million

Disease and case management programs and generic drug use reduced medical expenses by $69 million in 2006, according to a report by HealthPartners Inc. The programs include services that reduced medical complications for members with chronic illness and serious behavioral health conditions.

"We know that providing care that prevents medical complications significantly improves health and quality of life and now we have data to show that it also reduces the medical costs and helps us make care more affordable," said Dr. George Isham, HealthPartners’ chief health officer.
