Welcome to the Medical Home Neighborhood

During a May 15th webinar, Care Compacts: Forming the Foundation of PCP/Specialists Care Teams, Robert Krebbs, director of payment innovation at WellPoint, Inc., will share the key components of WellPoint's medical neighborhood pilot. The pilot, which launched in January, includes three specialties with clear care coordination alignment opportunities with primary care medical homes. The pilot includes practices ranging in size from solo practices to large group practices. With the help of Care Compacts (also known as Care Coordination Agreements and/or Referral Agreements), WellPoint is driving accountability between primary care physicians and specialists in this pilot program.

In this pre-conference interview, Mr. Krebbs describes the PCMH-N education needs on the part of specialists and patients who have not been part of integrated delivery systems so they can better benefit from the new patient-centered medical world. Listen online at:
