Care Coordination in 2016: Though Oversight
of Complex Comorbid Spans Continuum,
Reimbursement Remains Uneven

White Paper Abstract: Care coordination involves deliberately organizing patient care activities and sharing information among all vested participants to achieve safer and more effective care, per the Agency for Healthcare Research and Quality (AHRQ). For the 86 percent of respondents to a September 2016 Care Coordination survey who reported formal care coordination initiatives, those patient care activities span all care settings, including the patient's home.

While 70 percent of respondents' care coordination occurs in outpatient or ambulatory settings, 53 percent coordinate care in the hospital inpatient arena, and 41 percent in post-acute care facilities. While care coordination happens telephonically for 85 percent of respondents, more than three-quarters see patients one-on-one for this purpose.
