These rules apply to the Healthcare Intelligence Network Healthcare Toolkits Award contest conducted by the Healthcare Intelligence Network, 1913 Atlantic Avenue, Suite F4, Manasquan, NJ 08736. By completing this survey, you agree to be bound by these rules:

1. To enter: Every entry form submitted with a toolkit constitutes one entry in the contest. All complete entries must be postmarked by July 31, 2006.
2. To win: The Healthcare Intelligence Network will select 1 contest winner from all valid entries recorded. Winners will be notified by email or phone immediately after selection. Should a winner fail to claim the prize within five (5) days after notification from the Healthcare Intelligence Network, the prize is forfeited by such Winner and an alternate winner will be chosen.
3. Prize: The prizes to be awarded through the Healthcare Intelligence Network Contest include the following: one iPod Shuffle--- (approx. value of $99) Winner may not choose alternative prize. Prizes are not transferable.
4. Winner: The Healthcare Intelligence Network will notify Winner by e-mail or telephone. As a condition to claiming the prize, the Winner must agree: to be bound by these Rules and by the decisions of the Healthcare Intelligence Network.
5. Eligibility: The contest is open to lawful residents of the United States who are 18 years of age or older. Electronic entries must be submitted by U.S. residents logging on to the Internet from within the United States. Employees, contractors, and agents of the Healthcare Intelligence Network and their immediate family members are not eligible to participate in this contest.
6. Use of entrant information: The Healthcare Intelligence Network may contact entrants in the future with inquires about mutual business interests. Entries become property of the Healthcare Intelligence Network.
7. Entries will not be returned.
8. Entries constitute permission to use a review of the toolkits in future editorial products developed by the Healthcare Intelligence Network.
9. Miscellaneous: Subject to all applicable laws. Void where prohibited. The odds of winning are determined by the number of entries received. The Healthcare Intelligence Network may discontinue this contest at any time. The Healthcare Intelligence Network may terminate this contest or decide to modify it in any manner which it deems fair and equitable to participants if it becomes aware of any occurrence which the Healthcare Intelligence Network finds would materially affect the contests' security or fairness. The Healthcare Intelligence Network may prohibit individuals from participating in contest and disqualify entries if they (1) attempt to enter the contest through any means other than as described in these Rules, (2) attempt to disrupt the contest or circumvent the terms and conditions of these Rules in any way or (3) repeatedly show a disregard for the contest rules or act (a) in an unsportsmanlike manner, or (b) with an intent to annoy, abuse, threaten, or harass any other entrant or the Healthcare Intelligence Network. The Healthcare Intelligence Network is not responsible for late, lost, misdirected, mechanically reproduced, misdelivered, incomplete, illegible, or unintelligible entries, messages, or mail; unavailable network connections; failed, incomplete, garbled or delayed computer transmissions; online failures; hardware, software or other technical malfunctions or disturbances; or any other communications failures or circumstances affecting, disrupting or corrupting the contest in any manner including without limitation, omission or deletion of entries, destruction of entries, receipt of entries, entrant's ability to participate in the contest, winner notification, winner response to notification, or unauthorized access to or alteration of entries, whether any of the foregoing is inadvertent or intentional. The Healthcare Intelligence Network is also not responsible for any damage to entrant's computer occasioned by participation in the promotion or downloading of any information necessary to participate in the promotion. Incomplete, non-conforming, or illegible entries will be disqualified.
10. All entries are the property of the Healthcare Intelligence Network. The term "entries" used herein shall include notification. The Healthcare Intelligence Network's decisions with respect to the contest are final. This contest is subject to all applicable federal and state laws.
