New Chart: Top 5 Embedded Case Manager-Patient Interactions

SUMMARY: Despite challenges ranging from physician and practice buy-in to recruitment and retention, the number of healthcare companies embedding or co-locating case managers in care sites continues to rise. We wanted to see how embedded case managers interact with their patients and members.

HIN's Healthcare Case Management e-survey conducted in January 2012 captured the efforts and contributions of case management from 153 healthcare organizations, and also drilled down to trends in embedded case management. According to survey respondents, the top five ways embedded case managers interact with patients and members are:

  • Face to face in care setting: 90.9 percent

  • Telephone: 79.5 percent

  • Home visits: 40.9 percent

  • Provider visits: 40.9 percent

  • E-mail: 38.6 percent
